Workers' Compensation

The Workers’ Compensation program at the Âé¶¹´«Ã½Ó³»­ is administered by the State Office of Risk Management (SORM).  All Âé¶¹´«Ã½Ó³»­ employees who are injured in the course and scope of their employment are eligible to file a Workers’ Compensation claim. 

All work-related injuries must be reported to Risk Management.  Risk Management will file all claims with SORM.

Employees must complete the following forms:

  • Employee Report of Injury
  • Employee Leave Election
  • Authorization to Release Information
  • Witness Form (if applicable)
  • Spanish Employee Report of Injury
  • Spanish Leave Election
  • Spanish Authorization to Release Information
  • Spanish Witness Form (if applicable)

Network Physician

Employees who elect to pursue a Workers’ Compensation claim, must seek treatment from a physician within the Network of Physicians designated by SORM and administered by CareWorks.  A list of network physicians can be found here:

Pharmacy Benefit

Injured employees who elect to pursue a Workers’ Compensation claim are eligible for pharmacy benefits when their network physician writes them a prescription.  MyMatrixx administers the pharmacy benefit.  

Injured employees need to present a First Fill form to the pharmacy.  Risk Management can provide this form upon the filing of a Workers’ Compensation claim.
