Incident Reporting

When to Report

In the laboratory, incidents involving the injury of yourself or others may occur. The number one priority during these moments is to seek help immediately. Ensure that you and others are safe before resuming other activities such as submitting an incident report.

An incident includes:

  • Injuries

  • Near-misses (no injury, but risk was present)

  • Equipment malfunctions

  • Any chemical or biological spills, even small ones

Reporting process:

  1. Address immediate safety concerns and seek medical help if needed.

  2. Complete the Incident Reporting Form (paper forms available in each lab) and submit via email to LabManagement@untdallas.edu  or deliver a physical copy—do not duplicate.

  3. For biosafety incidents, also submit a Biohazard Incident Report Form.

Only faculty members and instructors are responsible for filling these out, not students. All incidents must be reported using the Incident Report Form.
